Diagnosis Related Group (DRG) systems were introduced in Europe to increase the transparency of services provided by hospitals and to incentivise greater efficiency in the use of resources invested in acute hospitals. In many countries, these systems were also designed to contribute to improving – or at least protecting – the quality of care. After more than a decade of experience with using DRGs in Europe, this book considers whether the extensive use of DRGs has contributed towards achieving these objectives. Written by authors with extensive experience of these systems, this book is a product of the EuroDRG project and constitutes an important resource for health policy-makers and researchers from Europe and beyond. The book is intended to contribute to the emergence of a ‘common language’ that will facilitate communication between researchers and policy-makers interested in improving the functioning and resourcing of the acute hospital sector. The book includes: A clearly structured introduction to the main ‘building blocks’ of DRG systems An overview of key issues related to DRGs including their impact on efficiency, quality, unintended effects and technological innovation in health care 12 country chapters - Austria, England, Estonia, Finland, France, Germany, Ireland, the Netherlands, Poland, Portugal, Spain and Sweden Clearly structured and detailed information about the most important DRG system characteristics in each of these countries Useful insights for countries and regions in Europe and beyond interested in introducing, extending and/ or optimising DRG systems within the hospital sector

Gina Domingo lives in the world of southern California pop culture. Cuban-American by birth, Gina is less a multiculturalist than an omniculturalist, absorbing everything in her path. But her life with her overly-protective parents takes an unexpected turn when GinaÍs paternal grandmother, Estela, visits. Here is GinaÍs abuela„whom Gina hasnÍt seen since the family left Cuba„come to set the record straight. Now GinaÍs entire range of experience, memories, and family truths begin to change. Estela doesnÍt impose her history on a family still coming to grips with its past and life in exile. Instead, she regales her granddaughter with tales of the island. When Estela unexpectedly dies while visiting the United States, Gina finds she has been bequeathed a legacy of freedom to create her own memories, her own version of the past. Like SalingerÍs Holden Caulfield, or the heroines of Joyce Carol OatesÍs Foxfire, the teenaged protagonist of Brand New Memory is possessed of a voice so simpàtica„so engrossing in her perception of herself, her family, and her friends„that we find ourselves mesmerized and unable to stop turning the pages.

Ardently infused with the theme of exile, Diary of Fire tells the story of political refugee Camilo MacIas, who, as a boy, flees Cuba with his parents in 1969 to settle in Los Angeles. Narrated as memories from a present of loss--a fire has consumed Camilo's home--the novel weaves together journal entries, poems, letters, photos, and excerpts from the character's first published work of fiction. Ingenuously, Camilo sees those scattered writings as pieces of a puzzle that will some day be completed, thus showing him "the big picture." Key issues emerge from his narration: the sexual and physical abuse Camilo suffered as a child and the resulting trauma, his conflicted relationship with both communist Cuba and the Cuban exile communities in the United States, his bisexual nature, and the strained relationship he has with his father. Ultimately, Diary of Fire relates a story of survival. Events unfold as passages from a memoir. as he paints a self-portrait of a bisexual Cuban American writer in exile, Camilo strives to build a life out of fear and cruelty and ashes, but also out of hope.

This book looks at how an innovation in health care financing and administration, which originated in the United States, was greeted by numerous western European countries. In eleven original chapters, European experts offer in-depth examination of the rate, degree, and manner in which their countries adapted to Diagnosis-Related Groups (DRGs), an innovation adopted by the U.S. government to create incentives for increased hospital efficiency. Comparing and contrasting the varying levels of resources and activity committed to this innovation, this book provides a range of insights for those involved in managing and promoting innovation. In The Migration of Managerial Innovation, health care experts from Belgium, Denmark, England, France, Germany, Norway, Portugal, Switzerland, and Sweden offer brief descriptions of the way health care is organized and financed--and a detailed examination of the fate of DRGs--within each of their countries.
