This book contains the most up-to-date scientific information on the anatomy, physiology, and pathophysiology of the bladder muscle, nerves, and collagen in the patient with bladder exstrophy. In addition, this book covers the most modern surgical treatment of the infant born with bladder exstrophy and those who fail initial reconstruction. Chapters are contributed by major children's centers throughout the world. The aims of this book are to familiarize both the resident and the experienced practitioner with the most modern methods for the treatment for bladder exstrophy and epispadias. This book deals with the function, innervation, and pathophysiology of the bladder muscle in the patient born with bladder exstrophy. New concepts from leading laboratories around the world regarding the function of the bladder are discussed. In addition, experts offer their advice, expertise, and thoughts concerning the management of the patient born with bladder exstrophy. This book is intended for researchers who are interested in both the innervation, structure and function of bladder muscle and for pediatricians, pediatric surgeons, pediatric orthopedic surgeons, and both adult and pediatric urologists who deal with patients from birth until adult life who are born with bladder exstrophy. This is the first book to deal specifically with the basic science of bladder exstrophy and the clinical management of the bladder exstrophy-epispadias complex.

The undertaking of the treatment of an infant born with bladder exstrophy is one of the most weighty responsibilities that can fall upon the shoulders of the reconstructive sur geon. The modern treatment of a child born with bladder exstrophy began in the mid- 1970's with the widespread application of staged reconstruction. This approach has consistently yielded very good results in several series. However, as in all serious congeni tal birth defects, there is certainly room for advancement. Issues such as the routine use of osteotomy, timing and type of epispadias repair, combining bladder closure with epispadias repair, the approach to the small bladder, and the management of a failed exstrophy still remain. This National Institute of Health/Na tional Kidney Foundation/Johns Hopkins-sponsored seminar was an attempt to bring ex perts in the field of pediatric orthopedic surgery, pediatric urology, pediatric surgery, adult urology, and basic science together to share their experiences in an attempt to foster new clinical and basic science research communications between the participants. If these col laborations result, then this first international meeting will have been successful. The editors would like to thank all of the contributors for their timely and complete submissions. The second, fully updated edition of this book applies and contextualizes up-to-date information on pediatric surgery for low and middle-income countries (LMICs). The book is organized in general anatomic and thematic sections within pediatric surgery, such as urology, oncology, orthopedics and gastroenterology and includes chapters addressing the unique challenges and approaches for pediatric surgery in low-resource settings. Each chapter has dual authorship LMIC author providing context-specific insights and authors from high-income countries (HICs) contributing experience from well-resourced settings. Written in a reader-friendly format, this book has a uniform structure in each chapter, with introduction, demographics, etiology, pathophysiology, clinical presentations, investigations, management, outcome, prevention, ethics, evidence-based surgery and references. This comprehensive volume fills the gap between up-to-date pediatric surgical scholarship and knowledge developed and applied in HICs, and the practical needs of practitioners in low-resource settings. This is an indispensable guide for postgraduate surgical trainees in Africa and other LMICs as well as general surgeons practicing in Africa and other LMICs, who need to care surgically for children.

This third edition of Pediatric Urogenital Radiology has been thoroughly updated to take account of the recent advances in the imaging and treatment of pediatric nephro-urologic disorders that have been achieved over the past years. A number of new chapters have been included on topics such as the role of ultrasound and MRI for urogenital imaging in the fetus and the use of contrast media in childhood. Other chapters have been extensively revised or rewritten, while information that continues to be pertinent has been retained. The book describes in detail all aspects of pediatric urogenital radiology. It is written primarily from the point of view of the radiologist, but also includes essential clinical information from and for the pediatrician, pediatric surgeon, and urologist. It is specifically designed to aid the clinician in making decisions on imaging management, and to help the radiologist to understand the clinical background and needs. The newest techniques and the changing relevance of imaging and interventional procedures are described, and the diverse problems associated with the changing anatomy, physiology, and pathophysiology from the newborn period to adulthood are explained. The whole spectrum of imaging features of agenesis, anomalies and malformations, dysplasia, parenchymal and cystic diseases, urolithiasis, neoplastic diseases, renal vascular hypertension, renal failure, renal transplantation, pre-and postoperative imaging, and genitourinary trauma is covered. Individual chapters are devoted to vesicoureteric reflux, urinary tract infection, congenital urinary tract dilatation, upper urinary tract dilatation, voiding dysfunction, and neurogenic bladder. A chapter on the clinical management of common nephrourologic disorders explains how imaging is embedded in the whole process of clinical management. Short conclusions are included at the end of chapters and sections to highlight the key information.

Pediatric and Adolescent Urologic Imaging provides a comprehensive reference for health care providers of children and adolescents with urologic conditions. This is the first book in which each chapter is written collaboratively by at least one author from each specialty. This unique approach melds the expertise of each specialist and offers it to the reader in a manner aimed at reinforcing the integration of clinical information to radiologic imaging. The book is arranged into two sections allowing for easy access to the information. The first section covers the principles of each radiologic modality as well as radiation safety and the history of uroradiology. The second section integrates the lessons of the first section into specific urologic conditions arranged anatomically and includes additional unique conditions. Pediatric and Adolescent Urologic Imaging is a key reference for pediatric urologists and radiologists as well as primary care providers, general urologists and radiologists, fellows, residents, medical students, and mid-level providers.
