La Unidad de Cirugía Bariátrica y Metabólica del Hospital Universitario Ramón y Cajal cuenta con una experiencia acumulada en más de 900 casos de cirugía de la obesidad en los últimos 6 años, y realiza un volumen mínimo anual de 120 casos, fundamentalmente Gastrectomía vertical (Sleeve) y Bypass gástrico, así como de Cirugía de revisión tras fallo de otros procedimientos, siendo uno de los grupos más punteros a nivel nacional. La Unidad nació por el interés en esta patología de los Servicios de Cirugía General y Digestivo (Dr. Eduardo Lobo) y de Endocrinología y Nutrición (Dr. Héctor Escobar), y ha sido coordinada y desarrollada por el Catedrático de Cirugía de la Universidad de Álcala de Henares, Profesor Pedro Carda. En los últimos años ha cedido la coordinación al Dr. Ignacio Botella del Servicio de Endocrinología y posteriormente al Dr. Julio Galindo del Servicio de Cirugía. Los excelentes resultados conseguidos en cuanto a pérdida de peso y resolución de comorbilidades en este tipo de pacientes, se han logrado gracias a la colaboración de los distintos servicios implicados en la Unidad (Cirugía General, Endocrinología y Nutrición, Psiquiatría, Cirugía Plástica, Anestesiología y Gastroenterología) , así como al alto grado de especialización en cuidados de Enfermería tanto en quirófano, dado que es una cirugía laparoscópica avanzada, como en los cuidados pre y postoperatorios.

Health systems should function in such a way that the amount of inappropriate care is minimized, while at the same time stinting as little as possible on appropriate and necessary care. The ability to determine and identify which care is overused and which is underused is essential to this functioning. To this end, the "RAND/UCLA Appropriateness Method" was developed in the 1980s. It has been further developed and refined in North America and, increasingly, in Europe. The rationale behind the method is that randomized clinical trials--the "gold standard" for evidence-based medicine--are generally either not available or cannot provide evidence at a level of detail sufficient to apply to the wide range of patients seen in everyday clinical practice. Although robust scientific evidence about the benefits of many procedures is lacking, physicians must nonetheless make decisions every day about when to use them. Consequently, a method was developed that combined the best available scientific evidence with the collective judgment of experts to yield a statement regarding the appropriateness of performing a procedure at the level of patient-specific symptoms, medical history, and test results. This manual presents step-by-step guidelines for conceptualising, designing, and carrying out a study of the appropriateness of medical or surgical procedures (for either diagnosis or treatment) using the RAND/UCLA Appropriateness Method. The manual distills the experience of many researchers in North America and Europe and presents current (as of the year 2000) thinking on the subject. Although the manual is self-contained and complete, the authors do not recommend that those unfamiliar with the RAND/UCLA Appropriateness Method independently conduct an appropriateness study; instead, they suggest "seeing one" before "doing one." To this end, contact information is provided to assist potential users of the method.

Can human social evolution be described in terms common to other sciences, most specifically, as an energy process? The Eighth Day reflects a conviction that the human trajectory, for all its uniqueness and indeterminism, will never be satisfactorily understood until it is framed in dynamics that are common to all of nature. The problem in doing this, however, lies in ourselves. The major social theories have failed to treat human social evolution as a component of broader natural processes. The Eighth Day argues that the energy process provides a basis for explaining, comparing, and measuring complex social evolution. Using traditional ecological energy flow studies as background, society is conceived as a self-organization of energy. This perspective enables Adams to analyze society in term of the natural selection of self-organizing energy forms and the trigger processes basic to it. Domestication, civilization, socioeconomic development, and the regulation of contemporary industrial nation-states serve to illustrate the approach. A principal aim is to explore the limitation that energy process imposes on human social evolution as well as to clarify the alternatives that it allows. Richly informed by contemporary anthropological historicism, sociobiology, and Marxism, The Eighth Day avoids simple reductionism and denies facile ideological categorization. Adams builds on work in nonequilibrium thermodynamics and theoretical biology and brings three decades of his own work to an analysis of human society that demands an extreme materialism in which human thought and action find a central place.

Physical movement has a positive effect on physical fitness, morbidity, and mortality in individuals with diabetes. Although exercise has long been considered a cornerstone of diabetes management, many health care providers fail to prescribe it.
