Annotation. Our nation faces the distinct possibility of a catastrophic terrorist attack using an improvised nuclear device (IND), according to international and U.S. intelligence. Detonation of an IND in a major U.S. city would result in tens of thousands to hundreds of thousands of victims and would overwhelm public health, emergency response, and health care systems, not to mention creating unprecedented social and economic challenges. While preparing for an IND may seem futile at first glance, thousands of lives can be saved by informed planning and decision making prior to and following an attack. In 2009, the Institute of Medicine published the proceedings of a workshop assessing the health and medical preparedness for responding to an IND detonation. Since that time, multiple federal and other publications have added layers of detail to this conceptual framework, resulting in a significant body of literature and guidance. However, there has been only limited planning effort at the local level as much of the federal guidance has not been translated into action for states, cities and counties. According to an informal survey of community preparedness by the National Association of City and County Health Officials (NACCHO), planning for a radiation incident ranked lowest in priority among other hazards by 2,800 local health departments. The focus of Nationwide Response Issues After an Improvised Nuclear Device Attack: Medical and Public Health Considerations for Neighboring Jurisdictions: Workshop Summary is on key response requirements faced by public health and health care systems in response to an IND detonation, especially those planning needs of outlying state and local jurisdictions from the detonation site. Hospitals and healthcare facilities are not able to singularly mitigate the patient surge from a mass-casualty incident or pandemic health event. The potential volume of patients demands that regional healthcare communities be able to respond as a unified body to maintain the resiliency of their healthcare systems. The National Healthcare Preparedness Program advocates the establishment of fully functional, response-ready regional healthcare coalitions to meet this need. Establishing a regional healthcare coalition requires that an appropriate governance structure be established, a proper level of participation be solicited, and adequate funding mechanisms be put in place. This thesis offers a case study of how these factors influence the ability of three existing and distinctively different healthcare coalitions to prepare for a patient surge from a mass-casualty or pandemic health event. The thesis also shows the influence of each of the factors on a coalition's sustainability. In recent years, many urban areas have established healthcare coalitions composed of autonomous (and often competing) hospitals, with the goal of improving emergency preparedness and response. We study the role of such coalitions in the specific context of response to multiple-casualty incidents in an urban setting, where on-scene responders must determine how to send casualties to medical facilities. A key function in incident response is multi-agency coordination. When this coordination is provided by a healthcare coalition, responders can use richer information about hospital capacities to decide where to send casualties. Using bed availability data from an urban area and a suburban area in the United States, we analyze the response capability of healthcare infrastructures under different levels of coordination, and we develop a stress test to identify areas of weakness. We find that improved coordination efforts should focus on decision support using information about inpatient resources, especially in urban areas with high inter-hospital variability in resource availability. We also find that coordination has the largest benefit in small incidents.This benefit is a new value proposition for healthcare coalitions, which were originally formed to improve preparedness for large disasters.
