This Atlas summarizes current surgical strategy for cerebral revascularization in the treatment of complex neurovascular diseases. It focuses on complex intracranial aneurysms, which are mostly large/giant, irregular and short of enough collateral compensation. In the first part, it starts from the extracranial-intracranial (EC-IC) bypass strategy. For the complex middle cerebral artery aneurysms, the types of EC-IC bypass are determined based on the angioarchitecture. Furthermore, various intracranial-intracranial (IC-IC) bypasses are introduced, with the advantage of no need for graft vessel harvesting and preferable matching of donor and receipt arteries. This Atlas provides useful knowledge and cases about this basic and indispensable skill for neurosurgeons.

Most cerebrovascular diseases such as intracranial aneurysms (IA), arterio-venous malformations (AVM) or dural fistulas (AVF) can effectively be treated with microsurgical techniques (i.e. clipping) or with endovascular procedures (i.e. coiling). In certain patients, also gamma-knife (radiation) treatment may play a role. Sometimes, the treatment strategy may consist of a staged approach that combines some of the above-mentioned techniques in a serial manner. Likewise, also spinal vascular pathologies (i.e. spinal vascular malformations, spinal dural AV-fistulas or spinal cavernomas) may often be approached with a single treatment modality or, more rare with a staged approach. In selected cases however, simultaneous treatment combining open (micro-)surgical and endovascular procedures in one single hybrid operation may be favorable. So far only few series reported indications, potential benefits, or pitfalls with hybrid approaches in cerebrovascular surgery. This collection aims to summarise reported experiences with hybrid approaches that combine simultaneous endovascular and microsurgical interventions in a setting of vascular diseases in the brain and in the spine. It will put an emphasis on indication, benefits and potential harms of combined diagnostic-therapeutic procedures (i.e. intraoperative control DSA after clipping), advanced endovascular strategies enabled through surgical access (i.e. direct arterial punctuation after previous surgical vessel exposure) or combined therapeutic operations (i.e. proximal control through balloon occlusion during clipping).

Seven Bypasses: Tenets and Techniques for Revascularization is the third book in a trilogy of bravura, technical nuance, and strategy by master neurosurgeon Michael Lawton. Like his first two books on aneurysms and AVMs, Seven Bypasses provides unparalleled firsthand insights and guidance on complex pathologies in vascular neurosurgery. The fundamentals of microsurgical anastomosis and the craft of bypass surgery are explored in depth with clinical pearls in every chapter. Lawton eloquently reveals the art of cerebral revascularization in exquisite, metaphorical detail. The surgeon performing bypass surgery is like an architect envisioning and building a beautiful structure. A bypass is designed to fit the patient's unique anatomy; blueprints designate anastomotic sites, connections, and conduits; the anastomoses are constructed; and the bypass is brought to life with pulsations, flow, and reperfusion. The book highlights Lawton's aesthetic, which has evolved from the common STA-MCA bypasses to IC-IC bypasses and elaborate arterial reconstructions. Key Highlights Stepwise discussion of the three anastomoses that form the building blocks of all bypasses: end-to-side, side-to-side, and end-to-end anastomoses Ten tenets delineate nuances of bypass: dexterity, preparing donors and recipients, establishing a working zone, temporary arterial occlusion, arteriotomy, suturing technique, tissue handling, knot tying, patency, and aneurysm occlusion Step-by-step guidance on the seven bypasses: EC-IC bypass, EC-IC interpositional bypass, arterial reimplantation, in-situ bypass, reanastomosis, IC-IC interpositional bypass, and combination bypass Strategies and algorithms for aneurysms organized by specific anatomical sites, including the MCA and the Sylvian cistern, ACA and the interhemispheric cistern, basilar artery and the basal cisterns, and PICA and the cisterna magna More than 1,500 radiographs, operative photographs, and exquisite illustrations drawn by artist Kenneth Xavier Probst elucidate anatomy, surgical principles, and clinical cases Dr. Lawton has bequeathed a remarkable treasure of knowledge to current and future generations of neurosurgeons and their patients. The Seven series is destined to be an enduring classic for residents, fellows, and neurosurgeons specializing in the treatment of cerebrovascular disease, and for those who believe that manual dexterity and technical skill still matter. Michael T. Lawton, MD, is Chairman, Department of Neurological Surgery; Professor of Neurological Surgery; President and Chief Executive Officer, Barrow Neurological Institute; Chief of Vascular and Skull Base Neurosurgery Programs; and Robert F. Spetzler Endowed Chair in Neurosciences, Barrow Neurological Institute and St. Joseph's Hospital and Medical Center, Phoenix, Arizona, USA. In July 2004 specialists in neurosurgery, neuroradiology, neurology and neurointensive care discussed recent trends at the 2nd Swiss Japanese Joint Conference on Cerebral Stroke Surgery, held in Zurich, Switzerland. New concepts were worked out during the conference and are published in this volume. The book starts with the topic intracranial aneurysms, discussing microsurgical and endovascular treatment modalities, as well as new surgical approaches. Further chapters deal with the management of unruptured aneurysms and with subarachnoid hemorrhage. Practical guidelines for vasospasm treatment are given. Together with contributions about arteriovenous malformations and fistulas, cerebral revascularization techniques and surgery related to the intracranial venous system a comprehensive overview about stroke surgery is given with an interdisciplinary approach. The book will be of interest for all specialists involved in therapy of cerebrovascular disease.

This volume provides an overview of new concepts in neurovascular interventions based on clinical and scientific knowledge of cerebrovascular disorders. It especially focuses on subarachnoid hemorrhage and cerebrovascular malformations, e.g. aneurysms, arterio-venous malformations, and cavernomas. A separate part addresses cerebral revascularization for both complex aneurysms and ischemia. All contributions were written by recognized experts and cover original papers presented at the 7th European Japanese Stroke Surgery Conference, held in Verona, Italy in June 2014. The authors present new trends and strategies for managing emerging problems, as well as in-depth discussions on controversial issues in the field.
