At age 53, I was diagnosed with advanced prostate cancer. It was in June 2005 that my PSA was 23.5 with a Gleason of 7. All 12 cores of the biopsy had cancer, 2 up to 75%. I knew action had to be taken, but what would I choose? Based on the current trend in medicine, I decided to go the holistic route. My research concluded that I had over 95% probability of recurrence. Therefore, in my use of holistic treatment, in conjunction with traditional treatment I got tremendous results. But sadly 9 months after treatment all signs indicated my cancer was back. I was able to discover a holistic treatment that reduced my PSA 100%.

Prostate cancer is unusual among solid tumors in that the majority of affected men die with, rather than of, the disease. This presents many challenges to healthcare professionals and patients in terms of deciding if, when and how to intervene in order to control tumor growth and spread, thereby extending survival but without compromising quality of life. This is the ninth edition of 'Fast Facts: Prostate Cancer' since 1996, testament to the rapid changes in the field and the steadily improving outlook for patients. This new edition provides many key updates: • the Gleason grade grouping, which has valuable prognostic value • nomograms to evaluate risk • our rapidly expanding understanding of the genetics and underlying pathogenesis of prostate cancer and the development of genomic tests to help identify those at greatest risk of developing clinically significant disease • the continuing debate about the role of PSA in the screening, detection and monitoring of prostate cancer • advances in imaging techniques, particularly multiparametric MRI, which is improving the accuracy of biopsy and reducing the numbers of negative biopsies • the roles of drugs, surgery and radiotherapy in the treatment of prostate cancer at different stages, and our ever-improving understanding of when and how best to intervene, aided by improving understanding of the risk factors for disease progression. Primarily intended for primary care providers, specialist nurses, junior doctors and allied healthcare professionals, this highly readable resource provides a comprehensive overview of prostate cancer, enabling fully informed discussions with patients about this complex disease. Contents: • Epidemiology and pathophysiology • Diet, lifestyle and chemoprevention • Screening and early detection • Diagnosis, staging and prognostic indicators • Management of clinically localized disease • Managing recurrence after initial therapy • Management of metastatic prostate cancer • Management of castrate-resistant prostate cancer • Survivorship and treatment complications

Prostate cancer is the second most common cancer in men worldwide, but it is unusual among solid tumors in that many men die with, rather than of, the disease. This raises challenges in terms of deciding if, when, and how to intervene to control tumor growth and spread in order to extend survival without compromising quality of life. The many developments in the field since the last edition of this book include continuing studies to improve detection and monitoring of the disease, with the evaluation of MRI for screening and prostate-specific membrane antigen (PSMA)-PET/CT for staging. Treatment options have also continued to improve and expand for all stages of the disease with the development of ultrahypofractionated radiotherapy, oral LHRH antagonists, PARP inhibitors and lutetium-177 PSMA-617 radioligand therapy.

Although it is the most frequently diagnosed cancer in men, prostate cancer is highly misunderstood. The disease has little effect on many, yet it is deadly to others. The book is a form of expedition-an expedition to explore the scientific wildernesses of biology, chemistry, and physics of the mechanisms that promotes and fuels the deadly form of Prostate Cancer. Readers will journey through the modern understanding of the disease that physicians use to exploit its weaknesses, and that result in therapies to reduce or mitigate its devastation. The expedition is for those who want to dig deeper into the subject of advanced prostate cancer. The account is written in understandable terms for a layman, yet retains some scientific rigor without the numbing jargon used in evidence-based scientific and medical journals. The terrain covered is not the initial treatments of prostate cancer-watchful waiting or active surveillance, proton therapy, brachytherapy, external beam therapy, or radical prostatectomy. There are many good books on these topics. This book is primarily about when prostate cancer takes a turn for the worse, and becomes advanced. Most books stop, or say little, on this topic. We pop the hood and take a look at the inner workings of the disease-the science of the disease, its weak points that therapies attempt to exploit, and we examine some of the experiments that got us to our current understanding. This information is difficult to find in an easily digestible form. Books are less likely to be found on these topics as it is not "mass market," and frankly are much more difficult to write. Hundreds of scientific papers have to be researched to keep up with developments. It should appeal to those with the curiosity of a cat, or the inquisitiveness of a scientist. The magic of several beautiful experiments is explained. As such, prostate cancer patients, scientist, engineers, and biotechnologists who want to get an overview of the science applied to this important health menace will benefit from the expedition. Advanced prostate cancer is a disease that must be fought, and fought hard. Like a military fight, its afflicted must become warriors against the disease. Part of the war chest of a warrior is intelligence (information). We need information to synthesize if we are to produce wisdom. And an abundance of wisdom (and luck) is needed to be successful in the fight. It is not just a patient's physicians that lead the prostate cancer journey that a prostate cancer warrior takes, but instead it is a partnership between patient and physician. The expedition for the reader results in providing current information about the science of the disease so that the prostate cancer warrior becomes a kind of Captain Clark, who works side by side with his co-expedition physician leader Captain Lewis to explore the forbidding landscape of this disease.
